If you are not a student, PhD-student or recently graduated but you want to register at our practice, you can make an appointment via the assistant for an acquaintance. You should bring your insurance card and ID! Besides, you may print and fill in this declaration already:

(But if you don't have a printer, we have a declaration for you in the practice.)
If this consultation pleases reciprocally, you can register as a new patient. Afterwards, you should inform your former general practitioner about the change, and to have your medical file sent to our practice.
